1. Owner/Publisher Details
2. Proposed Title Details
3. Affirmation under PRP Act, 2023 – a declaration confirming that the periodical is not involved in any activities prohibited under the Press and Registration of Periodicals (PRP) Act, 2023
4. Acknowledgement of periodical’s content

Preparation:

• The affidavit must be printed on stamp paper.
• It must be notarized by a Notary Public to be valid for submission.
